Background Check Procedure for Families
Once a host family has accepted a student placement, all adults over the age of 18 living in the host family home
are required to submit to a state and national background check.
Homestay Florida is registered with Daon livescan fingerprinting services.
The fee is $49.50 per person. Their fingerprinting services and equipment are available at participating UPS Store locations.

Enter Homestay Florida’s ORI #: V06030155 (The “V” stands for Volunteer)
You’ll then see on the following page:

Under Reason for Request, select “Volunteer” and complete the rest of the page.
You’ll then make an appointment with a local UPS branch location, provide payment, and receive a confirmation.
Each adult will each have to complete this process separately (each create an account and make an appointment).

IRS Form W-9 Download
Once a family has completed its background check(s), it must provide us with IRS Form W-9 (“Request for Taxpayer Identification Number and Certification”).
Homestay Florida maintains the W-9 for verification purposes. The information on Form W-9, along with the homestay income that you receive from us, are reported to the IRS on Form 1099.
You can download a fillable copy of Form W-9 from the IRS website. Please complete, sign, and return to us via email. A completed W-9 is required prior to formal placement.
